Hi, I have experienced something very similar, in my case it was after I updated the ct63 to the latest firmware- bin file. It said something along the lines of ctcm corrupted, and proceeded to boot in 33mhz. This happend after using the bin file of Patrices, with the bios screen, to get round it I re-ran in the old bin file 1.03c from September and held down ctrl, alt, right shift and undo (I think) after loading in Didiers bin file to clear NVRAM. I then disconedcted power cable and held in power button to drain any power left in the ct63, then booted in 030 mode and reloaded in Patrices firmware. In 030 mode I loaded the ct60 cpx then changed the frequency from 66mhz back up 100mhz. I think somewhere along the line these different bin files do not like higher frequencies? for instance using Didiers september firmware the system did not like to be clocked over 97 whereas Patrices I could happily run for hours at 100-106mhz.
Maybe the firmware you were using caused this problem also? can you not try a different firmware first, if i remember correctly the new one does not like being clocked over a certain frequency after updating abe & sdr chips using the jtag cable? So if you can load 103c and set cpx to 66 MHz then proceed with chip update, if success then reflash the ct60 firmware to latest to allow for the new drivers, and pci bridge.
It was a nightmare for at least a few hours and can happily say (touch wood) it has not happend again!!
I'm yet to get my ctpci, hope you get yours fixed soon.
