I´m sure you was careful,it must not be your fault.
I once had an USB data-logger where one pin of an 8pin SMD-chip was bad (cold) soldered by factory. This was impossible to see, sometimes it worked,sometimes not.
I only found it because i took a wooden stick and tapped and pressed softly on the
different parts (with power on). Wood because plastic (or rubber) can produce static electricity.
