Those with device ID $4755 and $4756.
See for example http://pciids.sourceforge.net/iii/?i=1002 for some names of cards with different IDs.
IIRC, the Eclipse PCI BIOS will display the vendor and device ID information at startup. But you never get that far, of course...
I believe a PC is likely to display the ID during it's startup as well. Otherwise you'd most likely be able to find it from inside the device manager.

Yes, but then the problem would still have been likely to happen even without a PCI card plugged in, IMO.
Unless there is a short circuit between some unfortunate pins of the connector, I suppose.
You might want to try checking for short circuits between PCI connector pins.
Obviously a few of them are grounds and are supposed to be connected together, but most should not be.
