We had the same problem in Aranym. What is happening, is if you directly renders to the screen surface, on OSX, this surface will be copied back to system ram, then you modify it, and then it is copied back to video ram, hence very slow.
So now in Aranym, every rendering is done in a SDL_Surface in system ram, and then only changed parts are sent to the video ram, to speed up rendering.
