I have these very same issues.
TOS with NOTHING running and a dialog box kills the system when I try clicking on anything.
Overall completely unreliable.
I have replaced the NVRAM but it makes no difference whether I try the option in the CPX to replace the flash, it just does not know the Flash chip.
Again, like you I had to use 030 mode to get it to save with the CPX but of course I got the NOT AN 060 thing too!
NVDI 5 is a must there is no doubt about that, and my NVDI Disks are not too clever... I do have the already installed files backed up but I am now unable to do a fresh install and so I need that sorting out.
