The problem was indeed with the CTCM.  After I broke off two pins removing it (sigh), I replaced it with a 66MHz oscillator and the CT60 started right up. 
 
The CTCM will be sent back to Rodolph for repair, but frankly, 66MHz beats 16MHz any day.
