Just inversed IOR & IOW in labels declarations.
Let me time to control the timings and you will have to update the CPLD of the CTPCI, what you already need without IDE ...
I have 140ns access cycle and I should down remove 20 cycles to get the ATAPI standard PIO MODE4 120ns cycle.
