I have good news. Problem is solved.
My collegues found the error. There was a trace on the sv extender that was broken during installation (It must have happened after the test when the extender was plugged in and before or during soldering).
So, thanks to all of you on dhs who provided help (and to my collegues who are real professionals. Usually they do industrial work, soldering smd etc. so the falcon was a nice job variation for them).
